What Does the following MRI Indicate? 

i am diagonis ARTHRITIS , done a two MRI 1st report says... 28*09*2012 that says MR findings reveals mild fusion and sclerosis along the inferior part of bilateral sacroiliac joint with marrow edema along the facet joint and acetabular margings of bilateral hip joints . possibility of inflammatory arthritis is most likely.,,,,,,, seond MRI done on Straightening of lumbar spine is seen. there is evidence of triangular T1W, T2W hyperintensities seen along the margins of vertebral bodies likely Romanus corner s sigh likely due to Ankylosing spondylitis. Posterior and foraminal disc bulge is seen at L4/5 level causing thecal sac indentation with mild compression on bilateral exiting nerve roots... kindly suggest what to do enable to sit and walk

Hello,

The MRI report is indicative of secondary changes via Ankylosing Spondylitis condition involving lower lumber discs and thecal sac causing indentation, narrowing of the intervertebral space and underlying nerve terminals compression. Management depends upon multiple parameters and common ones are as follows:
- Local application of heating pad over lower back
- Analgesic spray or ointment containing diclofenac, linseed oil for lower lumbar area
- Analgesic and anti inflammatory medicines after meals
- Control of body weight and Body mass Index accordingly
- Control of blood sugar and cholesterol
- Assisted physiotherapy with an expert physiotherapist

Consideration of extent of progress in symptomatic relief after thorough practice of above for a period of 3-6 weeks and later if required one may have to be ready for surgery if no significant outcome via conservative approach.
